MOTOR VEHICLE COLLISION INSURANCE
Have you been injured in a motor vehicle collision?
- Report the accident to your insurance company. You will be required to complete an AB-1 Form, which must be submitted within 10 business days of your accident. The form can be provided to you by your insurer; if not, we have the form available for you.
- You may choose whether you would prefer a Physiotherapist or a Chiropractor to be your primary health care practitioner (who will complete your initial assessment and manage your care plan)
- Come in for an initial assessment within 10 days of your accident (if possible).
- Our staff will help you fill out the insurance forms needed (AB-1 and AB-2)
- Following your assessment, your practitioner will submit a comprehensive report to your insurance company (AB-2). Once your claim is authorized, your therapist will let you know how many treatments are approved (based on your diagnosis) and your insurance will cover the treatment costs
- Your insurance will also cover the cost of exercise equipment or supports your practitioner recommends
